Eligibility
The Passport Office at UC San Diego only accepts applications for U.S. Passports. You must be a U.S. Citizen and show proof of citizenship to apply.
Application Requirements
- DS-11 application filled out in black ink and UNSIGNED
- Shorten your appointment time by completing the DS-11 in advance.
- Do not sign the application until instructed to do so by your assigned passport agent.
- Visit the U.S. Department of State's passport page to download the DS-11 application form.
- Proof of U.S. Citizenship: Original or Certified copy of Birth Certificate, Certificate of Naturalization, or a previous passport.
- Proof of Identity: Valid Driver's License, State-issued ID, Official U.S. military ID card, or Government employee ID card.
- If the Driver's License or State-issued ID is issued outside of California, another form of photo identification is required. (e.g., Costco card, expired ID, school or work ID, etc.).
- Two separate forms of payment are required*
- Application fee - Check or Money Order only
- Passport office fee - separate form of payment (Card, Cash, Check, Money Order)

Additional Requirements for Minors
Ages 16-17
- At least one parent must be present
- Parent and Minor must bring proof of identity.
Ages 15 and under
- Both parents must be present and bring proof of identity
- If a parent can't be present, then parental consent is required (See below).
- Proof of U.S. Citizenship: Original or Certified copy of Birth Certificate, Certificate of Naturalization, or a previous passport.
- A birth certificate is required, in addition to a prior passport.
