Learn about the eligibility requirements to apply for a U.S. passport, including application requirements for minors

The Passport Office at UC San Diego only accepts applications for U.S. Passports. You must be a U.S. Citizen and show proof of citizenship to apply.

- Our agents can assist you in filling out the application during your appointment. However, filling it out in advance will speed up the process. Do not sign the application until instructed to do so by your assigned passport agent.
- Visit the U.S. Department of State's passport page to download the DS-11 application form.
Proof of U.S. Citizenship: Original or Certified copy of Birth Certificate, Certificate of Naturalization, or a previous passport.
Proof of Identity: Valid Driver's License, State-issued ID, Official U.S. military ID card, or Government employee ID card.
- If the Driver's License or State-issued ID is issued outside of California, another form of photo identification is required. (e.g., Costco card, expired ID, school or work ID, etc.).
Two separate forms of payment are required*
1. Application fee - Check or Money Order only
2. Passport office fee - separate form of payment (Card, Cash, Check, Money Order)

Each applicant must have two forms of payment. *Visit the fees page for details.

Both parents must be present and bring proof of identity
- If a parent can't be present, then parental consent is required (See below).
Proof of U.S. Citizenship: Original or Certified copy of Birth Certificate, Certificate of Naturalization, or a previous passport.
- A birth certificate is required, in addition to a prior passport.
